Graduate Program Forms & Policies

View Forms & Policies for graduate students matriculated in the Pharmaceutical Sciences Graduate Program. While all provisions stated in the University Graduate Handbook apply to Pharmaceutical Sciences graduate students, this on-line repository contains unique programmatic policies, forms, and instructions that supersede the University Graduate Handbook. Pharmaceutical Sciences graduate students are required to use these forms for documenting adequate progress towards the intended degree according to the following timelines:

International Student Fee

All students are required to pay the International Student Fee. The fee is $150 per semester ($300 per year). The fee covers the costs associated with SEVIS compliance, optional practical training (12 month and STEM OPT), processing, onboarding for campus employment, an international resource center, airport transportation, cultural programming, orientation and the mandatory health screening.
