Our program prepares residents to fill positions as clinical providers of advanced patient care services in communitybased pharmacy settings with a focus on underserved communities.

Purpose

PGY-1 pharmacy residency programs build on Doctor of Pharmacy (PharmD) education and outcomes to develop communitybased pharmacist practitioners with diverse patient care, leadership, and education skills who are eligible to pursue advanced training opportunities including postgraduate year two (PGY-2) residencies and professional certifications.

Equipping Residents

  • Provide culturally appropriate direct patient care services
    65% of your time is spent providing care to patients within our practice sites. You will be involved in one-one and group patient appointments for chronic disease state management, provide life-saving immunizations, help patients during transitions from hospital/nursing home to home and provide medications during patient centered dispensing activities.
  • Hone skills essential to becoming an integral part of a health care provider team
    You will work with a variety of other health care providers- doctors, nurse practitioners, dieticians, nurses and medical assistants as part of a team providing care to the underserved of our community. While in the pharmacy, you will lead both day to day operations as well as be involved in broader planning at the organizational level developing leadership and team work skills.
  • Educate patients/family members, health care providers, health professions and the community
    As part of your experience, you will be educating a variety of learners: pharmacy students in large lectures, small groups and skills lab modules as well as on APPE rotations; patients and caregivers in individual and group settings and community members in health promotion/education presentations as well as other health care professionals. At the end of the residency year, you will have completed all the requirements for a teaching certificate.
  • Develop leadership, research and health care improvement skills needed to advance community-based practice
    You will complete 3 year-long preceptor mentored projects that help advance the practice of pharmacy. From implementation and evaluation of a new clinical service to improving workflow processes to creating a business plan to justify a new service, each project will be tailored to the needs of the practice site. Each project will be presented to a larger audience at national, state and local meetings.

Saint Vincent De Paul Logo

At St. Vincent de Paul Charitable Pharmacy, you are engaged in providing advanced disease state management services under collaborative practice agreements with area physicians. You will lead one-on-one appointments for comprehensive medication reviews and robust medication therapy management services. You will work with our on-site nurse practitioner to run 11 different point-of-care labs and create recommendations. The patients you care for are uninsured or underinsured who require a provider who is compassionate and creative in order to help them improve their health. MATCH 216415

As a consultant pharmacist, you will be managing over 600 patients by reviewing their charts monthly to help manage a variety of disease states while building relationships with providers.  Your patients may be in long-term care or assisted living settings or in group homes. It is your job to balance treatment with quality to ensure optimal health outcomes for your patients. MATCH 216423

Five Rivers is a community care pharmacy participating in the 340b drug pricing program providing care to the underserved of their community.  You work closely as a member of the care team alongside providers (both physicians and nurse practitioners), nurses, medical assistants and other health care professionals to provide comprehensive care.  Working under collaborative practice agreements and within the electronic medical record, you see patients for one-on-one and group appointments to manage chronic disease states such as diabetes, hypertension and hyperlipidemia, balancing priorities to improve overall health. CHD has a Federally Qualified Health Center (FQHC) designation status and serves over 40,000 patients annually.  CHD operates six Primary Care Health Centers that include 340B pharmacies, one free-standing dental center, one free-standing vision and dental center and thirteen School-Based Health Centers.   

Our six pharmacies are full-service Cincinnati Health Department dispensaries. These pharmacies serve the medical needs of patients in the community. MATCH 214621

What Residents Say

For many patients at St. Vincent de Paul Charitable Pharmacy, we are providing care to someone who may not receive it otherwise. I was counseling a patient recently on how to use his rescue inhaler, then he used one puff while I was with him. This was a great learning experience for both of us! I noticed that he was breathing out immediately after inhaling the dose. I learned that actually seeing how the patient uses their device is more helpful that just talking through it. I have interactions like this on a daily basis with our patients. I truly feel that I am making an impact in our patients’ lives and optimizing their care.
Every day I work with internal medicine residents, family medicine residents, behavioral health professionals and other medical staff at Five Rivers Health Center daily to provide patient care and chronic disease state management in an underserved patientpopulation. I have learned the importance of interprofessional team work to patient care and have improved my communication skills with both medical professionals and patients. The residency hasprovided me with a well rounded experience in community based pharmacy practice.
As a resident with Medication Managers, I visit nursing homes and long-term care facilities to review patient charts and evaluate medication profiles. There are always new things to learn from the charts—it can be a patient with a rare disease or a medication being used for a unique diagnosis. Long-term care is a different environment from other healthcare settings. Seeing consultant recommendations impact patient care is one of the most rewarding parts of this residency.

A letter of intent, Curriculum Vitae, college transcripts, and three letters of recommendation are required. Candidates whose completed application materials are received by December 31 of the application cycle will be considered.
